NOMINATION FORM

Please refer to the Nomination Guidelines for information about the Award and questions regarding author eligibility. 

  • Authors may nominate themselves or be nominated by a publisher, friend or fan.
  • Fields marked with an asterisk(*) are required. 
  • If you are nominating an author whose work has been self-published, two chapters of the author’s work must be attached as a PDF file to the form. Authors that have had a book traditionally published do not need to submit chapters of their work. Click here for a list of publishing houses we consider self-publishing companies.
  • Nominations must be received by Friday, March 16, 2018, in order to be considered. 

Eligibility criteria:

  • Authors must be born in Indiana or have lived in Indiana for five consecutive years in their lifetime.
  • Author must be living and able to attend the Award Dinner to accept their award.
  • Authors must be willing to participate in outreach activities in the year following the Award (such as author visits, panel discussions or taped question and answer sessions). These appearances will be paid and we will work with the author on what activities work with their schedule and availability.
